Fall 2013 Registration
ALL Fall 2013 WALK-IN REGISTRATIONS
HAVE CONCLUDED.
We have concluded all in-person registration events and are now ONLY accepting registrations via Mail. All players registering by mail will be placed on a Wait List and assigned to a team with a coach on a space available basis. We are always looking for coaches... if interested, please consider taking a coaching certification course. The course is required for coaching in AYSO.
Our MAIL-IN REGISTRATION INSTRUCTIONS:
1) Go to www.eAYSO.org and log-in. (Contact info kaneoheayso.org OR 235-12976 or

2) Complete a New or Returning player registration form and/or complete a Volunteer Form (mandatory for U5 parents).
3) Print 1 copy of the player registration form.
(Note: New for this year is e-Signature. Be sure your electronic signature is accepted before trying to print. NOT e-signing will prevent us from officially registering your child. Sign your printed copy (for medical release for Coaches) Volunteer forms also need 1 signed copy along with photocopy of I.D.
Hints for successful printing (print from eAyso log in home page under Print Player Form):
- Click on the printer icon shown next to your child’s name to print form. DO NOT CLICK ON THE BROWSER PRINT ICON. Correctly printed forms prints on 2 pages or 1 page double-sided.
4) Indicate your child’s uniform size at the top of the form (just write it in anywhere at the top of the form). Uniform sizes offered are as follows:
· YXS – Youth Extra Small * AS – Adult Small
· YS – Youth Small * AM – Adult Medium
· YM – Youth Medium * AL – Adult Large
· YL – Youth Large * AXL – Adult Extra Large
5) Mail the following items to the address shown below.
· 1 copy of Player registration form (use the age guide shown below to determine your child’s divison
· Copy of your child’s birth certificate (NOT necessary if your child played in the Kaneohe AYSO fall 2012 season)
· Check made payable to AYSO Region 113. Send one check per child if registering multiple children.
PLAYER FEES:
$88 for players registering in the U-6 to U-19 division **Please note that your check WILL NOT be
$73 for players registering in the U-5 division deposited until we place your child on a team.

Detailed Player Registration Instructions:
NEW PLAYERS (New to soccer or have never played in the Kaneohe Region):
1. Go to www.eayso.org or follow the link on this page and create a user account.
2. On the left hand side of the Home page go to Application for New Player & input the required information.
3. When completed, print and sign 1 copy of the registration form. In order to print, you must go back to the Home Page of eayo.org and click on Print player form on the left side. (Note: the registration form may print on 2 pages instead of 1. That’s OK.)
4. All NEW players, whether new to soccer OR new to the Kaneohe area, must also bring a Birth Certificate or other proof of age.
5. Mark the top of each form with your child’s uniform size.
6. Mail all 1 copy of your child’s player registration form, and the required proof of age, i.e. Birth Certificate and payment, to AYSO REGION 113, Post Office Box 404, Kaneohe, HI 96744.
REGISTRATION IS NOT COMPLETE until forms are signed & turned in.
RETURNING PLAYERS (played in Kaneohe in the FALL 2012 SEASON or Previous Years):
1. Go to www.eayso.org or follow the link on this page. Log-in with username and password if you have one OR create a user account by clicking MY child has played AYSO, but we have never used eayso and follow all instructions.
2. On the left hand side of the Home page, go to Application for New Player & input the required information.
3. When completed, print and sign 1 copy of the registration form. In order to print, you must go back to the Home Page and click on Print player form on the left side.
(Note: the registration form may print on 2 pages instead of 1. That’s OK.)
4. If you have previously used www.eayso to register your child online, on the left side of the Home page, go to Application for RETURNING PLAYER and fill in required information.
Note: you may update address,e-mail and phone numbers at any time throughout the year by going to UPDATE Player on the home page.
5. When completed, print and sign 1 copy of the registration form. In order to print, you must go back to the Home Page of eayo.org and click on Print player form on the left side. (Note: the registration form may print on 2 pages instead of 1. That’s OK.)
6. Mark the top of each form with your child’s uniform size.
7. Mail all 1 copy of your child’s player registration form, and the required proof of age, i.e. Birth Certificate & Payment to AYSO REGION 113, Post Office Box 404, Kaneohe, HI 96744.
Players who did not play the Fall 2012 season must include copy of birth certificate.
REGISTRATION IS NOT COMPLETE until forms are E-signed, printed & turned in.
NOTE: AT LEAST ONE U5 PARENTS MUST FILL OUT A VOLUNTEER FORM. A VOLUNTEER FORM COMPLETED ONLINE WILL ENSURE THAT YOUR PERSONAL INFORMATION REMAINS PRIVATE & SECURE. WE THANK YOU FOR YOUR UNDERSTANDING.
VOLUNTEER REGISTRATION AYSO is an ALL volunteer organization.
1. Go to www.eayso.org or follow the link on this page. Log-in with username and password if you have one OR create a user account.
2. Look on the right hand side of the home page (Adults wanting to volunteer)and go to Apply as a new volunteer if you are new. Input the required information. If you have previously volunteered, go to Apply as a Continuing Volunteer and update your information as needed.
3. When completed, print and sign 2 copy of the volunteer form along with photocopy of I.D.
4. Mail in with your Player Form and payment.
REGISTRATION – Frequently Asked Questions
1. My son took a break last year from AYSO soccer in Kaneohe and did not play. Is he considered a returning player?
No. He is considered a new player and he will have to be registered as a new player. You will need to mail in a copy or email (ayso.113.registrar@gmail.com) of his birth certificate so that we can verify his age.
2. How do I contact someone if I have questions about the online form or have problems?
